SACRAMENTO - California is spending billions of state dollars on benefits available to some undocumented residents.

A City Journal investigation published Sept. 23 says the state spends at least $11 billion annually on programs serving undocumented immigrants and their U.S.-born children, citing health care, cash aid, housing, tax credits, college assistance, phone discounts and low-cost auto insurance. It also gives a hypothetical mixed-status family whose combined benefits could exceed $100,000 a year.

The biggest number is real, but most of it is health care. California’s Legislative Analyst’s Office estimates comprehensive Medi-Cal coverage for undocumented residents costs about $10 billion annually from the General Fund and covers roughly 1.7 million people. A California Department of Finance figure cited in reporting used by City Journal put the state-funded share at about $11.2 billion for that fiscal year.

That does not mean every program named in the investigation is part of one audited $11 billion pot.

Federal Medicaid generally does not pay for full coverage for undocumented adults beyond limited emergency and pregnancy-related services. California chose to finance broader coverage with state money. Since Jan. 1, 2026, however, most adults without satisfactory immigration status can no longer newly enroll in full-scope Medi-Cal. Dental benefits for most affected adults ended July 1, and a $30 monthly premium for certain existing adult enrollees is scheduled for July 2027.

Other programs work differently. CalWORKs may provide “child-only” aid when a parent is ineligible because of immigration status, meaning the eligible child receives the benefit. Federal Section 8 rules allow mixed-status households to receive assistance, but the subsidy is prorated according to the number of household members with eligible immigration status.

CAPI is narrower. California says applicants generally must be aged, blind or disabled, meet income and resource rules, satisfy specific immigration criteria, and be ineligible for SSI solely because of immigration status.

Some state programs expressly reach undocumented residents. Qualified students may receive California Dream Act aid, and CalEITC allows taxpayers to qualify using an ITIN. California’s Low Cost Auto program is available regardless of immigration status, but drivers still pay premiums. California LifeLine provides discounted phone service and is funded through a telecommunications surcharge, not the state General Fund.

One program frequently included in the debate is not yet in effect. California’s planned food-aid expansion for income-eligible residents 55 and older regardless of immigration status is scheduled for Oct. 1, 2027.

The fiscal issue is substantial, but the details matter. California has chosen to spend state money beyond federal immigration-based eligibility limits. Whether a household receives cash, medical coverage, a prorated housing subsidy or another benefit depends on separate eligibility rules.
