Inventia Healthcare Limited has quietly launched a voluntary recall of 11,460 bottles of Chlorthalidone Tablets USP, 25 mg, after the medication failed critical dissolution tests — raising serious concerns that the tablets may not break down correctly inside patients’ bodies, potentially reducing or eliminating their effectiveness.

The affected product, distributed by Rising Pharma Holdings Inc. in East Brunswick, New Jersey, comes in both 100-count and 1,000-count bottles under lot numbers RISA24001 and RISB24002, with an expiration date of April 2027. Chlorthalidone is a commonly prescribed diuretic used to treat high blood pressure and fluid retention associated with heart failure, kidney disease, and liver issues.

As of now, the recall has not received an official FDA classification, and no accompanying agency press release has been issued. Questions about potential health risks for patients taking the defective tablets remain unanswered.

This marks the second blood pressure medication recalled in just one month for the same dissolution failure. Earlier in May, Ascend Laboratories pulled 15,696 bottles of Amlodipine and Olmesartan Medoxomil Tablets (5mg/40mg) for identical reasons.

For patients relying on these medications to manage serious cardiovascular conditions, the idea that their pills might simply pass through the body without releasing the active ingredient is deeply alarming. Dissolution failures can lead to inconsistent or inadequate treatment, potentially putting people at higher risk for complications like uncontrolled hypertension or worsening fluid retention.

While no injuries have been reported so far, the lack of clear FDA guidance leaves many wondering exactly what steps they should take. Experts strongly advise against suddenly stopping blood pressure medication without consulting a doctor or pharmacist, as abrupt discontinuation can be dangerous.

This wave of recalls highlights growing quality control concerns in the pharmaceutical supply chain, especially for widely used generic drugs. Patients who have these specific lots are urged to check their bottles immediately and contact their healthcare provider for guidance on safe alternatives.
