Texas Children’s Hospital Ordered to Open Nation’s First Detransition Clinic in Landmark Settlement

It took three years, more than five million documents, and a legal siege by both the Texas attorney general and the United States Department of Justice to bring America's largest children's hospital to the negotiating table. On Friday, Texas Children's Hospital emerged from that table with a settlement that will reshape its identity in ways that would have seemed unthinkable when the investigation began — including an order to establish what officials are calling the first detransition clinic in the country's history.

Texas Attorney General Ken Paxton announced the agreement Friday, framing it as a seismic institutional reckoning. The settlement requires Texas Children's to terminate five physicians who previously provided gender transition care to minors, pay $10 million to resolve allegations that the hospital improperly billed Texas's Medicaid program for that care, and operate the new detransition clinic free of charge for its first five years.

"This settlement reflects an institutional and fundamental cultural shift away from radical 'gender' ideology," Paxton said in a statement.

A Hospital Under Siege — and Its Defense

Texas Children's Hospital, which serves as the largest pediatric medical institution in the United States, did not concede wrongdoing. In a statement that carried the measured language of an institution absorbing a blow while refusing to acknowledge it as a defeat, the hospital said it had cooperated fully throughout the investigation — producing more than five million documents and conducting multiple internal reviews — and maintained it had followed all applicable laws throughout.

"Today, we made the difficult decision to settle with the Texas Attorney General and the Department of Justice, closing a chapter that has been wrought with falsehoods and distractions," the hospital said. "To be clear — we are settling to protect our resources from endless and costly litigation. This settlement will allow us to redirect those precious resources to focus on the life-saving care and groundbreaking discoveries of our exceptional clinicians and scientists."

The hospital declined to identify the five terminated physicians, saying only that "our top priority throughout this settlement discussion was around protecting our physicians." It also offered limited detail on what the detransition clinic will provide, with a spokesperson describing it as a formalization of "the supportive, multidisciplinary services we already deliver to all patients who need our care." Who will lead or staff the new clinic remains unspecified.

What Detransition Care Actually Involves

The medical terrain of detransitioning — the process of no longer identifying as transgender or discontinuing a medical gender transition — is less charted than its political prominence might suggest. Research indicates it is rare: studies estimate that between 1% and 10% of transgender people detransition, and fewer than 1% report regretting their transition. The most frequently cited reasons for detransitioning include parental pressure, harassment or discrimination, and the practical difficulty of continuing the process.

The medical interventions involved in detransition care closely mirror those associated with transition itself — mental health therapy, hormone treatments, and in some cases surgery. Officials have not detailed precisely what services Texas Children's new clinic will offer beyond those broad categories.

The Investigation's Origins: A Political Campaign Meets a Legal Strategy

To understand Friday's settlement, one must trace it back to a methodical, multi-year campaign by Paxton's office to eliminate gender transition care for minors in Texas entirely. In early 2022, after the state legislature failed to pass a formal ban on such care, Paxton issued a legal opinion declaring transition care for minors to constitute child abuse — a designation that triggered state investigations into parents who had sought such treatment for their children.

The following year, Texas enacted what became the largest state-level ban on gender-affirming care for minors in the country. A few months before that ban took effect, Paxton set his sights directly on Texas Children's, publicly accusing the institution of "actively engaging in illegal behavior" by continuing to provide gender transition procedures to young patients.

"Under my watch, I will investigate and bring the full force of the law against any Texas hospital that abuses children with harmful medical interventions to 'transition' kids," Paxton declared at the time.

The DOJ's Expanding National Investigation

Friday's resolution carries significance well beyond the borders of Texas. The settlement marks the first concluded case in the Justice Department's sweeping national investigation into what the department has termed "sex-rejecting procedures" for minors — a probe that has extended its reach across the country with escalating speed and scope.

Last year, federal prosecutors subpoenaed more than 20 doctors and clinics treating transgender minors, alleging potential healthcare fraud and false statements. This week, NYU Langone confirmed it had received a grand jury subpoena from federal prosecutors requesting six years of records related to its treatment of transgender youth.

Acting Attorney General Todd Blanche left no ambiguity about the administration's intentions. "The Justice Department will use every weapon at its disposal to end the destructive and discredited practice of so-called 'gender-affirming care' for children," Blanche said Friday. "Today's resolution protects vulnerable children, holds providers accountable, and ensures those harmed receive the care they need."

The Human Dimension: Detransitioners and Their Advocates

The political momentum behind Friday's settlement has been partly shaped by a small but prominent cohort of detransitioners who have become public advocates for restrictions on transition care for minors. Figures including Chloe Cole and Prisha Mosley have filed lawsuits against the physicians who treated them, arguing that access to care was too easily granted and that medical providers failed in their duty. The doctors named in those cases have denied wrongdoing.

Their voices have amplified a policy debate that medical and scientific consensus has not settled in the direction that Paxton and the Justice Department claim. The research on regret and detransition rates remains a contested battlefield — and the establishment of the first government-mandated detransition clinic in American history ensures that battlefield will only grow more contested in the months and years ahead.

For Texas Children's Hospital, Friday was the end of one chapter. For the broader fight over gender care for minors in America, it may be only the beginning.
