World Health Organization declares Ebola outbreak in Congo and Uganda a global health emergency

Congo-Uganda Ebola outbreak

It began in the mining camps of eastern Congo, spreading quietly through communities where workers arrive and depart in constant rotation, carrying with them everything — including, now, a virus with no vaccine and no proven cure. On Sunday, the World Health Organization declared the Congo-Uganda Ebola outbreak a public health emergency of international concern — the organization's highest alert designation — as the death toll climbed to 87 and the virus demonstrated it would not respect national borders.

WHO Director-General Tedros Adhanom Ghebreyesus was careful to calibrate the alarm. The outbreak, he stressed in a Sunday statement, "does not meet the criteria of pandemic emergency" — and he urged countries explicitly against closing their borders, a measure that historically impedes the flow of medical personnel and supplies more than it stops a virus already in motion.

But the declaration itself is a flare fired into the global public health sky — a signal that what is happening in Ituri province in the Democratic Republic of Congo has outgrown a local response.

How It Started — and How It Crossed a Border

The outbreak was first officially reported Friday in the DRC's eastern Ituri province, though health officials now believe it began spreading as early as late April — weeks before anyone recognized it for what it was. That delay is precisely what makes Dr. Craig Spencer, a Brown University School of Public Health professor who himself survived the Zaire strain of Ebola in 2014 while working with Doctors Without Borders, so alarmed.

"This means we're just learning about this outbreak long after it's already been spreading," Spencer wrote on X following the announcement. "This makes it harder to find contacts and all the cases." In the language of outbreak response, every day of undetected spread is a day of lost contact tracing — a widening pool of invisible exposure chains that investigators must now race to map.

Most of the 336 suspected cases so far have been concentrated in two mining towns, Mongwalu and Rwampara — communities defined by high human mobility, where workers flow in and out constantly, creating near-ideal conditions for viral transmission to propagate unseen.

"We are talking about a region that is a very vulnerable and fragile region," said Dr. Jean Kaseya, director general of Africa CDC, during a Saturday video press conference.

The Man Who Carried It to Kampala

The outbreak's most alarming development arrived on May 14, when a 59-year-old Congolese man died from Ebola in Kampala — Uganda's capital city and a major regional hub. His journey tells the story of how a hemorrhagic fever moves in a connected world: he traveled from the DRC, landed in Uganda, took public transportation, moved through a community, fell ill, and was surrounded by an unknown number of people before reaching a hospital.

"Someone came from DRC, landed in Uganda, went to hospital," Kaseya recounted. "He was sick in this community and he was surrounded by a number of people. He took public transportation to Uganda." After dying in the hospital, his body was transported back across the border into the DRC for burial — each leg of that journey a potential exposure event that investigators are now working urgently to reconstruct.

Kaseya said he did not yet know what protective equipment, if any, was used by those who came into contact with the man during his illness and death — a gap in knowledge that represents one of the outbreak's most pressing unknowns.
