It began in the mining camps of eastern Congo, spreading quietly through communities where workers arrive and depart in constant rotation, carrying with them everything — including, now, a virus with no vaccine and no proven cure. On Sunday, the World Health Organization declared the Congo-Uganda Ebola outbreak a public health emergency of international concern — the organization's highest alert designation — as the death toll climbed to 87 and the virus demonstrated it would not respect national borders.

WHO Director-General Tedros Adhanom Ghebreyesus was careful to calibrate the alarm. The outbreak, he stressed in a Sunday statement, "does not meet the criteria of pandemic emergency" — and he urged countries explicitly against closing their borders, a measure that historically impedes the flow of medical personnel and supplies more than it stops a virus already in motion.

But the declaration itself is a flare fired into the global public health sky — a signal that what is happening in Ituri province in the Democratic Republic of Congo has outgrown a local response.

How It Started — and How It Crossed a Border

The outbreak was first officially reported Friday in the DRC's eastern Ituri province, though health officials now believe it began spreading as early as late April — weeks before anyone recognized it for what it was. That delay is precisely what makes Dr. Craig Spencer, a Brown University School of Public Health professor who himself survived the Zaire strain of Ebola in 2014 while working with Doctors Without Borders, so alarmed.

"This means we're just learning about this outbreak long after it's already been spreading," Spencer wrote on X following the announcement. "This makes it harder to find contacts and all the cases." In the language of outbreak response, every day of undetected spread is a day of lost contact tracing — a widening pool of invisible exposure chains that investigators must now race to map.

Most of the 336 suspected cases so far have been concentrated in two mining towns, Mongwalu and Rwampara — communities defined by high human mobility, where workers flow in and out constantly, creating near-ideal conditions for viral transmission to propagate unseen.

"We are talking about a region that is a very vulnerable and fragile region," said Dr. Jean Kaseya, director general of Africa CDC, during a Saturday video press conference.

The Man Who Carried It to Kampala

The outbreak's most alarming development arrived on May 14, when a 59-year-old Congolese man died from Ebola in Kampala — Uganda's capital city and a major regional hub. His journey tells the story of how a hemorrhagic fever moves in a connected world: he traveled from the DRC, landed in Uganda, took public transportation, moved through a community, fell ill, and was surrounded by an unknown number of people before reaching a hospital.

"Someone came from DRC, landed in Uganda, went to hospital," Kaseya recounted. "He was sick in this community and he was surrounded by a number of people. He took public transportation to Uganda." After dying in the hospital, his body was transported back across the border into the DRC for burial — each leg of that journey a potential exposure event that investigators are now working urgently to reconstruct.

Kaseya said he did not yet know what protective equipment, if any, was used by those who came into contact with the man during his illness and death — a gap in knowledge that represents one of the outbreak's most pressing unknowns.

No Vaccine, Scarce Protective Gear, and an Experimental Hope

The strain responsible for this outbreak is called Bundibugyo — a less common variant of the Ebola virus than the Zaire strain that devastated West Africa between 2014 and 2016, and consequently less well understood. There have been only two previous outbreaks of this strain. Critically, there is no approved vaccine.

The Africa CDC has identified a candidate experimental vaccine currently under study, but its limitations are stark. "We know it was only tested on some monkeys," Kaseya said, noting the vaccine has shown an efficacy rate of roughly 50% in animal trials. Its potential in human patients has not yet been assessed.

The absence of a vaccine compounds an already dire resource landscape. Healthcare workers treating Ebola patients require comprehensive protective equipment — head coverings, goggles, masks or face shields, multiple layers of gloves, full gowns, and rubber boots. Yet Kaseya acknowledged a critical vulnerability: "We don't have manufacturing for PPE." His team, he said, has identified funding needs and is working to address the shortfall — but in an active outbreak, procurement timelines measured in days can cost lives.

The Shadow of 2014

The numbers from this outbreak — 336 suspected cases, 87 deaths — exist against the long shadow of the worst Ebola outbreak in recorded history. Between 2014 and 2016, the Zaire strain ignited in Guinea and swept through Sierra Leone, Liberia, and beyond, ultimately infecting 28,600 people and killing 11,325 before the world's belated response brought it under control. That outbreak also began small, in a region considered remote, before the virus demonstrated it cared nothing for the assumptions of the people trying to contain it.

The Africa CDC said Sunday that it has been coordinating with health organizations, nonprofits, and pharmaceutical companies to reinforce cross-border surveillance, preparedness, and outbreak response across the region. WHO's emergency declaration unlocks additional international resources and formalizes the global response architecture.

The declaration of a public health emergency of international concern places the Congo-Uganda Ebola outbreak on the world's highest-priority infectious disease radar. It mobilizes resources, coordinates international response, and — critically — creates pressure on governments and donors to act before the outbreak's trajectory becomes irreversible.

What it cannot do, by itself, is stop the virus. That work falls to the healthcare workers moving through mining camps and border crossings in protective gear that may or may not be available, tracing contacts of a man who rode public transportation through a capital city, trying to find every link in a chain that started spreading before anyone knew to look for it.

The virus does not wait for declarations. It is already moving.
