Pentagon Will Test Service Members’ Testosterone Levels

In an administration that has made masculinity a governing philosophy as much as a cultural posture, the Pentagon has taken the ideology one step further — into the bloodstream. Defense Secretary Pete Hegseth announced Wednesday that the Department of Defense will launch a new testosterone deficiency screening program for US service members, ordering annual hormone tests for all troops aged 30 and older and opening the door for younger soldiers to opt in voluntarily.

The announcement, delivered in a video posted to X, positions the program as a medical readiness initiative — a tool for ensuring that the nation's fighting force operates at its biological peak on a battlefield Hegseth described as "brutal and unrelenting."

"I'm authorizing a new screening program for testosterone deficiency for our service members, ensuring you have the right testosterone levels to operate at your absolute best," Hegseth said. "By addressing these health markers early, we're keeping you on the leading edge of lethality, and giving you the same level of support that you give this nation — the absolute best."

What the Program Entails

Under the new initiative, service members Hegseth referred to as "warfighters" who are 30 years of age or older will undergo testosterone testing as part of their routine annual health assessments. Those under 30 may choose to participate on an opt-in basis. Any treatment that follows — including testosterone replacement therapy — is described as voluntary, with the stated aim of "restoring and optimizing" the natural hormonal capabilities of individual troops.

Hegseth framed the program in the language of long-term force readiness, stating that the initiative is designed to keep troops "strong and resilient" not just through their active service years but throughout their lives. He described it as part of the department's commitment to providing "elite medical care" — applying to military health what private performance clinics have been marketing to civilian men for years.

The program does not specify what testosterone levels will be considered deficient, what clinical thresholds will trigger treatment recommendations, or how the military's definition of hormonal adequacy will be established and standardized across its branches.

The Political Architecture Behind the Science

The Pentagon testosterone testing announcement does not exist in a vacuum. It is the military expression of a political preoccupation that has been building across the Trump administration — and the broader American right — for years.

Robert F. Kennedy Jr., the 72-year-old health secretary, has publicly discussed injecting testosterone as part of his personal anti-aging regimen and, in October, warned without scientific evidence that today's American teenagers possess "50% of the testosterone of a 65-year-old man" — a claim that drew immediate pushback from endocrinologists. Alternative media figures including Tucker Carlson have produced content decrying what they call a masculinity crisis in American culture. Online influencers actively promote what adherents call "T-maxxing" and push direct-to-consumer testosterone treatments to young men who may have no clinical need for them.

Research published in the journal Social Science & Medicine found that young men are being aggressively targeted online by influencers and wellness companies marketing hormone tests and treatments as essential to authentic manhood — despite the fact that screening for low testosterone is medically unwarranted in most people in that age group.

The testosterone fixation on the American right has constructed a political narrative in which declining hormone levels are both a symptom and a symbol of national decline — a framework that Wednesday's Pentagon announcement formally imports into US military policy.

The Contradiction at the Heart of the Policy

The ideological tension embedded in Hegseth's announcement is not subtle. The Trump administration has consistently and forcefully opposed the use of testosterone — along with estrogen — in gender-affirming care for transgender individuals, describing such treatments as "chemical and surgical mutilation" in a January 2025 executive order. The administration has simultaneously banned transgender service members from the military.

Wednesday's announcement authorizes the voluntary administration of the same hormone — testosterone — to cisgender male troops, framed not as mutilation but as optimization. The distinction the administration draws is one of purpose and identity, not pharmacology. The same molecule sits at the center of both policies, pointing in opposite political directions.

Hegseth's announcement also contains a conspicuous silence. It makes no reference whatsoever to the more than 231,000 women who serve as active duty members of the United States military — roughly 17% of the total active force. Whether female service members will have access to comparable hormone health screening programs, or whether the initiative is exclusively designed for male troops, was not addressed in the defense secretary's announcement or accompanying materials.

What Medicine Actually Says

The clinical picture on testosterone screening is considerably more nuanced than Wednesday's announcement suggests. Testosterone levels in men naturally decline with age — a gradual, normal physiological process that does not constitute a disorder in most individuals. Medical authorities including the American Urological Association have established specific clinical criteria for diagnosing hypogonadism — the medical condition associated with genuinely low testosterone — and those criteria involve both laboratory values and the presence of symptoms, not screening test results alone.

Broad population-level screening of healthy men for testosterone deficiency is not currently recommended by major medical organizations, in part because the benefits of treating subclinical low testosterone remain uncertain and the risks of unnecessary treatment — including effects on cardiovascular health, fertility, and the body's natural hormone production — are real.

What Comes Next

The Pentagon has not provided a timeline for when the testosterone screening program will begin, which branches of the military it will initially cover, or what infrastructure of clinical follow-up and treatment will be established to support it. The voluntary nature of any resulting treatment offers some protection against the most aggressive applications of the policy, but the mandatory screening component for troops 30 and older raises questions about how results will be used, documented, and factored into readiness assessments.

For an administration that has made the physical and cultural definition of masculinity a central political project, the Pentagon testosterone testing program is less a medical initiative than a statement — delivered in the language of military readiness, but carrying a message that extends well beyond the base.
