Roughly a million Americans rang in 2026 with an unwelcome task on their to-do list: find a new health insurer, fast. CVS Health's Aetna subsidiary walked away from the Affordable Care Act marketplace in 17 states, and at least a dozen additional carriers have since announced they'll stop selling plans next year — a wave that will push several hundred thousand more people into the same scramble.

A Predictable Unraveling, Not a Random One

These departures didn't happen in a vacuum. When enhanced federal subsidies expired at the end of 2025, premiums spiked, and healthier enrollees — the ones insurers rely on to balance out costlier patients — began dropping coverage in droves. What remained was a smaller, sicker risk pool for insurers to cover. One by one, companies looked at the math and walked away, because the numbers simply stopped working.

States Losing Ground Fast

Texas has lost three carriers over the past two years alone. Oregon is down two more. For the millions who depend on marketplace coverage, every departing insurer removes a competitive check that might otherwise keep prices from climbing even faster — a domino effect where fewer players means less pressure to stay affordable.

CVS Health Leads the Retreat

CVS Health became the first major insurer to exit, after its Aetna marketplace enrollees generated medical bills that outpaced what the company collected in premiums. CVS projected losses of up to $400 million on its ACA plans in 2025 — a staggering figure that made continued participation untenable. This marked the company's second retreat from the exchanges: Aetna first pulled out in 2017, returned in 2022, expanded further in 2023, and still couldn't make the business sustainable.

Cigna Follows Suit

Cigna announced its own exit in late April. The insurer's marketplace enrollment had already dropped 17% from the previous year, and president and incoming CEO Brian Evanko told analysts there was simply no viable path toward growth. Cigna will withdraw entirely from all 11 states where it currently sells plans, displacing roughly 369,000 enrollees, according to healthinsurance.org and Becker's Payer Issues.

Oregon: A Case Study in Shrinking Options

Oregon illustrates what happens when a state loses insurers it can't easily replace. PacificSource and Providence, which together cover nearly 96,000 enrollees across Oregon and neighboring states, are both exiting after confronting what Providence CEO Erik Wexler described as an "untenable situation" — neither company has enough enrollees to negotiate hospital rates competitive with those secured by larger national carriers. Oregon's marketplace will shrink from six insurers down to four, with no new carrier stepping in to fill the gap for 2027.

Texas Faces Its Own Contraction

Texas is grappling with a similar squeeze. Baylor Scott & White, a health system that also operates marketplace plans, will exit both the ACA marketplace and Medicaid entirely, according to healthinsurance.org, forcing roughly 100,000 enrollees to find new carriers.

Other insurers are making more selective retreats. Molina Healthcare is cutting its footprint by more than half, dropping from 14 states down to six. CareSource will exit Indiana, Ohio and West Virginia, affecting nearly 90,000 enrollees across those three states. Centene, meanwhile, is reshuffling its brand structure in Delaware, New Hampshire and Florida, shifting affected members between its various subsidiaries rather than exiting outright.

Smaller Players Feel the Squeeze Too

Mending, formerly known as Taro Health, is leaving the insurance business altogether. The startup plans to shut down operations and pivot toward direct primary care instead, leaving about 8,100 enrollees across Maine and Oklahoma without a carrier. Medica, ConnectiCare and Cox are also pulling back from Iowa, Kansas, Oklahoma, Connecticut and Missouri, according to KFF.

Why the Math Stopped Working

Even with net premiums jumping 58%, insurers that remain in the market still can't make the economics function. Cigna's Evanko told analysts in late April there was no realistic path to reversing enrollment that had already shrunk 17% year over year. Providence's Wexler pointed to consolidation among national insurers, arguing they've reached a scale that regional, not-for-profit plans simply can't match. PacificSource, for its part, said claims costs had outpaced what it could reasonably charge in premiums.

Premiums Set to Climb Again

Proposed 2027 premiums are on track for double-digit increases for the second year running, according to Georgetown's Center on Health Insurance Reforms — driven largely by the fact that enrollees who stuck around through 2026's initial price shock tend to use more care and cost more to insure. Higher premiums, in turn, risk pushing out more of the healthier enrollees insurers need to balance their books, creating a feedback loop that drives average costs even higher.

Federal Policy Adds to the Pressure

Washington isn't making the situation easier. Congress tightened the rules governing premium tax credit eligibility through H.R. 1, the reconciliation bill passed in 2025. That same year, CMS introduced new documentation requirements for anyone enrolling outside the standard open enrollment window. And CMS's finalized 2027 rule will raise out-of-pocket costs while adding more paperwork to the sign-up process — changes Georgetown's center estimates could push another 1.2 million to 2 million people out of the marketplace entirely.

What This Means for Coverage Going Forward

The exits announced so far won't disrupt coverage for the rest of 2026. People on expiring plans can maintain their current access through Dec. 31, as long as they keep paying premiums. But they'll need to secure a new insurer once the calendar turns to next year. Anyone who doesn't will automatically be placed on the cheapest available plan offering roughly comparable coverage.

A Shrinking Pool of Choices

That backup plan assumes options actually exist. In 165 counties, only a single insurer currently sells marketplace plans, according to KFF — up sharply from 93 counties just last year. Even in states where multiple carriers remain, the field is narrowing fast. Each departing insurer takes its provider network along with it, meaning that for many patients, losing a health plan could also mean losing access to a trusted doctor.

Five new carriers do plan to enter state marketplaces for 2027, but they won't come close to replacing what's being lost. The departing insurers built their negotiating leverage with hospitals and pharmacies on enrollment bases spanning multiple states. Colorado Access, one of the newcomers, will sell coverage in only part of a single state — nowhere near the scale needed to command the same favorable rates.
