Blueshield Downcoding Practice Threatens Patient Access to Mental Health Care

A single word — downcoding — has quietly upended how doctors and therapists across Chicago get paid. And for patients like Rachel, learning that Blue Cross Blue Shield of Illinois was quietly cutting her longtime psychiatrist's reimbursement felt like watching a lifeline start to fray in real time.

A Patient Springs Into Action

The moment Rachel discovered what was happening, she didn't sit still. She reached out to elected officials and state representatives, hoping someone could push back against the insurer's new claims policy — one that had slashed her psychiatrist's per-visit pay so severely that he was seriously weighing whether to drop Blue Cross Blue Shield patients entirely.

"I'm going to do absolutely everything I can do to avenge my wonderful psychiatrist," said Rachel, who asked that her last name be withheld, fearing professional blowback for speaking out.

A Legal Gap That Leaves Some Providers Exposed

Not every mental health provider is protected under Illinois' new downcoding law — and that gap is already causing real strain. Derek Phillips, director of psychiatry at Arcus Behavioral Health & Wellness in Chicago, falls outside the law's coverage as a prescribing psychologist. It's a rare professional category: prescribing psychologists conduct psychotherapy, administer psychological testing, and prescribe medication for conditions spanning anxiety to schizophrenia — yet there simply aren't many of them practicing.

In recent weeks, Phillips has watched dozens of his claims get reduced by Blue Cross Blue Shield, shrinking both his own earnings and those of the practice employing him.

A Small Practice, an Overwhelming Task

Fighting back isn't simple. Since each downcoded claim must be appealed individually, Phillips' small practice likely lacks the bandwidth to challenge them all.

"We only get paid based on what we're reimbursed by insurance companies," Phillips said. "So if they pay less, we make less" — a blunt equation that leaves little room to absorb the losses quietly.

'It's a Huge Access Issue'

For some providers, the financial squeeze created by lower reimbursement rates may prove unsustainable — and the timing couldn't be worse, arriving at a moment when finding a therapist is already a genuine struggle for countless patients.

Rachel said her own psychiatrist has told her and his other patients that he doesn't believe he can keep treating Blue Cross Blue Shield patients as long as the current policy remains in place.

Rachel Can Afford the Alternative — Many Can't

Rachel says she's prepared to pay out of pocket for her sessions, even at nearly $200 a visit. She typically sees her psychiatrist twice a year, and given the strength of their relationship, she has no plans to switch providers.

"I'm going to pay out of pocket to see him because I can," Rachel said. "And I have no intention of finding another psychiatrist that doesn't have years and years of my personal relationship with them. But so many people will not be able to pay out of pocket for this."

A System Already Stretched Thin

The broader numbers paint a troubling backdrop. A KFF analysis found that 32% of adults in Illinois who needed counseling or therapy weren't able to access it — a gap that Tony Ohlhausen, director of research and policy for the National Alliance on Mental Illness-Illinois, warns could widen further under the new claims practices.

"They're going to be less likely to take clients who have more complex needs if they're seeing those more intensive services be denied and only approved for lower rates," Ohlhausen said. "It will be making people's insurance policies less useful when it comes to mental health treatment."

A Pattern Playing Out Across Providers

Ohlhausen said NAMI Illinois has fielded concerns from both independent clinicians and larger provider groups alike, all grappling with the same wave of reduced claims — suggesting this isn't an isolated billing dispute, but a systemic shift rippling across the mental health care landscape.

When Algorithms Miss the Human Details

The impact isn't confined to psychiatry. Dr. Josh Levin, a suburban pediatrician, said roughly 40% of his practice's claims for higher-level sick visits were downcoded by Blue Cross Blue Shield in recent weeks. He argues the automated systems behind these reductions simply can't see what he sees in the exam room.

Algorithms that evaluate claims based solely on diagnosis codes, Levin said, miss the nuanced clinical judgment doctors apply daily — particularly when treating young children who can't always articulate what's wrong.

"In pediatrics, we are not only treating the medical problem, but also dealing with the fact that kids often can't tell us what's wrong with them. They can't speak about their symptoms, they may have developmental issues or behavioral issues," Levin said. "Distilling this down to a simple, 'This code or that code,' is really missing the whole picture of how pediatrics is practiced."
