Tennessee Prison Chief Resigns After Christa Pike Survives Failed Execution

Tennessee’s corrections chief is stepping down three days after the state attempted to execute Christa Pike and instead transported her to a hospital alive and critically ill, according to her attorneys.

Gov. Bill Lee announced Saturday that Tennessee Department of Correction Commissioner Frank Strada offered his resignation and will leave later this month. Strada said he still believes the department carried out its responsibilities “in accordance with the established protocol.” Lee has halted executions for the rest of 2026 and appointed former U.S. Attorney Ed Stanton to lead an independent review. Forth

That defense of the protocol may become more important than the resignation.

If investigators find employees departed from Tennessee’s rules, the focus will be on personnel, training and medical judgment. But if the state is correct that the written protocol was followed and Pike still survived two doses of pentobarbital, Tennessee faces a harder problem — whether the protocol itself can reliably prevent the kind of prolonged medical emergency that unfolded inside Riverbend Maximum Security Institution. Pike’s attorneys say execution personnel made at least seven needle attempts and failed to recognize that IV lines were improperly placed or had failed. Their court filing says pentobarbital entered surrounding tissue rather than properly flowing through her bloodstream, leaving both arms swollen, burned and blistered. They are asking a Davidson County court to require preservation of physical, written and electronic evidence from the execution attempt.

The warning signs were not entirely new. Pike’s lawyers had argued before the execution that her small veins and thrombocytosis could complicate IV access. Tennessee also experienced another IV-related execution failure on May 21, when officials abandoned the execution of Tony Von Carruthers after more than an hour of unsuccessful efforts to establish the required IV access. That makes the review about more than one bad night.

Pike, 50, was sentenced to death for the 1995 torture and murder of 19-year-old Colleen Slemmer. Slemmer’s mother, May Martinez, witnessed the failed execution and later said she did not receive justice for her daughter.

There is another issue many headlines miss. Surviving an execution attempt does not automatically cancel a death sentence. In Louisiana ex rel. Francis v. Resweber, the U.S. Supreme Court allowed Louisiana to try again after an electric-chair execution failed.

Pike’s facts, however, are different. Lethal drugs were actually administered, she allegedly suffered serious injuries, and her lawyers had warned about the IV risks beforehand. Any future effort to execute her again would be expected to trigger major Eighth Amendment litigation over whether another attempt would amount to cruel and unusual punishment.

Strada’s departure changes the leadership. It does not answer the question Tennessee now has to confront: did people fail the execution system, or did the execution system fail even when followed?
