The blue cross blue shield settlement payments are finally moving forward, as one of the largest antitrust settlements in U.S. healthcare history begins distributing funds to millions of Americans affected by alleged anti-competitive practices.

The massive $2.67 billion settlement stems from a long-running lawsuit accusing Blue Cross Blue Shield companies of quietly coordinating instead of competing—an arrangement plaintiffs argued inflated insurance costs like a pressure valve squeezing premiums higher. The insurer has denied all wrongdoing throughout the case.

Payments to roll out after years of legal battle

According to the settlement website, eligible customers who previously held Blue Cross Blue Shield health insurance plans are expected to begin receiving payments next month.

The case dates back to a 2013 antitrust lawsuit alleging that BCBS affiliates entered agreements not to compete with one another. In 2020, a federal judge in Alabama approved the settlement, which after legal fees and expenses left approximately $1.9 billion for class members.

Individual payouts will vary depending on coverage type and the amount of premiums paid during eligibility periods.

Millions of claims and strict deadlines

Roughly 6 million claims were filed nationwide. To qualify, individuals were required to submit claims before a November 2021 deadline, meaning late applicants are not included in the distribution.

The settlement class covers customers enrolled in Blue Cross Blue Shield plans between February 8, 2008, and October 16, 2020. It also includes certain healthcare providers who participated in administrative service plans and treated patients between July 24, 2008, and October 4, 2024. Providers were required to file claims by July 29, 2025.

Notices being sent as payments are prepared

The settlement administrator is currently reviewing submitted claims and sending notifications to claimants on a rolling basis. Emails and postcards began going out in February, signaling that distribution is entering its final stages.

Claimants who receive a notice are instructed to follow the provided steps carefully in order to secure their payment.

Industry changes tied to settlement

As part of the agreement, Blue Cross Blue Shield also committed to altering certain business practices aimed at increasing competition within the health insurance market. The changes are intended to open the door wider for competing insurers and reduce barriers within the system.
