A federal grand jury in Massachusetts has indicted Erekle Gugava, 33, on a money laundering conspiracy charge, accusing him of helping move proceeds from an alleged health care fraud scheme through U.S. banks and into overseas accounts.

The indictment was returned Thursday, Sept. 3, according to the Justice Department’s Sept. 4 announcement. Gugava is from the country of Georgia and was in the United States unlawfully, the department said.

Prosecutors allege that Gugava purportedly owned ND Medical Solutions LLC, a Pennsylvania durable medical equipment company, between February and July 2025. During that period, the company allegedly submitted at least $1.3 billion in fraudulent claims to Medicare, Medicare supplemental insurers, private employer-sponsored health plans and other insurers.

The $1.3 billion represents allegedly fraudulent billings. Insurers actually paid ND Medical approximately $6.5 million, according to prosecutors. That distinction separates the scale of the alleged billing effort from the payments identified in the government’s account.

Gugava’s alleged role centered on the banking side of the operation. Prosecutors say he opened several accounts for ND Medical, served as their sole authorized signatory and deposited reimbursement checks from insurers. The funds were ultimately transferred to overseas accounts for the benefit of a criminal organization based in Russia and elsewhere, the department alleges.

The government’s account describes a vulnerability that reaches beyond medical billing. Payments from Medicare and established insurance carriers initially appeared legitimate because of their source, prosecutors said, making the proceeds particularly susceptible to laundering.

For patients whose identities were allegedly stolen, the warning signs arrived in ordinary insurance paperwork.

Elderly and disabled Americans received explanations of benefits listing equipment they had never received, prescriptions from doctors they had never visited and a supplier they did not recognize. Many reported those discrepancies to Medicare and its contractors, according to the DOJ release. The allegedly misused identities belonged to people in Massachusetts, elsewhere in New England and across the country.

“Fraud networks cannot function without people willing to launder and transmit their proceeds,” said Assistant Attorney General Colin M. McDonald, who leads the National Fraud Enforcement Division. He said pursuing those facilitators is essential to protecting taxpayer resources.

DOJ linked Gugava’s case to the organization targeted by Operation Gold Rush, announced in June 2025. In that broader investigation, prosecutors alleged that a network used foreign straw owners to acquire medical supply businesses and submitted $10.6 billion in fraudulent Medicare claims using the stolen identities of more than 1 million Americans. Those figures describe the broader operation.

The Gugava investigation involves the HHS Office of Inspector General, the FBI, postal inspectors, IRS Criminal Investigation, Homeland Security Investigations and the Labor Department’s Employee Benefits Security Administration. Federal health care fraud prosecutors and the Massachusetts U.S. Attorney’s Office are handling the prosecution.

Gugava is charged with one count of money laundering conspiracy. The 20-year prison term is the maximum possible penalty if convicted. DOJ’s announcement did not include a response from Gugava or defense counsel.

An indictment does not establish guilt. Gugava is presumed innocent unless prosecutors prove the charge beyond a reasonable doubt in court.
