A simple billing error spiraled into a prolonged administrative nightmare for a Hillsborough County woman after she was charged for an ambulance ride she never took, in a case now highlighted as a troubling Florida ambulance insurance bill mistake involving merged medical records and insurance confusion.

The issue began when Gloria A. Valdes received an ambulance invoice belonging to another woman who shared her exact first and last name, setting off a chain reaction of insurance errors and disputed records that lasted more than a year.

Same Name, Wrong Patient, One Shared Account

The billing notice was addressed to Gloria Valdes without a middle initial, creating the first point of confusion.

Valdes told local outlet WFLA News Channel 8 that when she examined the details, the demographic information reflected someone else’s address but her own birth date.

Her insurance was incorrectly listed as secondary, while the other woman’s policy appeared as the primary payer. In effect, two separate ambulance records had been merged into a single account—like two different medical files stacked into one by mistake.

The billing contractor for Hillsborough County Fire Rescue, Digitech, processed the claim, and Valdes’ insurance initially paid for the other patient’s transport.

Dispute Turns Into Long Administrative Battle

When Valdes attempted to correct the error, she was told she would need to prove the bill was not hers.

That shifted the burden entirely onto her, while her personal insurance information remained entangled with another patient’s file—creating ongoing concerns about data accuracy and privacy.

The dispute stretched on for more than a year before media attention, led by journalist Shannon Behnken of WFLA, brought renewed scrutiny and helped resolve the issue.

How Medical Record Errors Happen

Experts say cases like this are not rare. A study by the Pew Charitable Trusts found that medical record mismatches occur most often in urban health systems, where patients frequently interact with multiple providers.

Common problems include duplicate records, split files, and simple data entry errors such as misspelled names or swapped identities.

These mistakes are not just administrative headaches—they can lead to incorrect billing, denied insurance claims, and in serious cases, wrong treatments or medication errors.

A 2026 study in BMJ Quality & Safety found that patients with duplicate records are about five times more likely to die after hospital admission than those with accurate, single records.

When Two Medical Files Become One

In Valdes’ case, the error was more severe than duplication. Her file was fully merged with another patient’s record, creating what experts describe as a “record overlay.”

Most patients are unaware this type of error can occur—or how difficult it can be to detect without careful review.

Patients Urged to Check Insurance and Medical Records

After insured care, patients receive an Explanation of Benefits (EOB), which outlines what insurers paid and what patients may owe. While not a bill, it can reveal mistakes such as duplicate charges or incorrect services.

Under federal law, patients have the right to access and correct their medical records, even if bills are unpaid. Providers must respond to correction requests within 60 days, with a possible 30-day extension, and notify other institutions if changes are made.

Health records can be requested through providers directly or via patient portals. Experts advise patients to carefully check for incorrect names, addresses, or medical histories, as well as inconsistent diagnoses.

Incorrect or mismatched personal details—such as unfamiliar contact information or conflicting medical histories—may signal deeper record errors or overlays.

Resolution After Public Attention

In Valdes’ case, no fraud was involved, but the consequences mirrored identity-related medical errors. With assistance from media coverage, the issue was eventually corrected and the records separated.

Her experience underscores how a small administrative mistake can ripple into a long-term financial and privacy burden, and why experts say vigilance may be the only protection against similar errors.
